Question: Which of the following questions would be appropriate in evaluating suidcie threat:

    A. Do you have suicidal ideation, a plan or a weapon?

    B. Do you know someone you can talk to?

    C. Have you discussed this with your parents?

    D. Do any of your friends know about your thoughts?

Correct Answer:  A. Do you have suicidal ideation, a plan or a weapon?

Rationale: The best assessment for suicide threat is to ask if the student has suicidal ideation, if they have a plan and if they have access to a weapon.  Recurrent thoughts of death, preoccupation to inflict injury or death, access to firearms are all characteristics of suicidal ideation.

Question:  You are the school nurse for a preK campus.  Upon registration, you note a student with their parents who tells you they tested positive for lead.  Which would be the best form of assessment for you as the school nurse to utilize for this student.

    A.  Send for blood levels to be drawn for lead.

    B. Observe for abdominal pain, nausea and/or pallor, fatigue and tachycardia indicating symptoms of anemia.

    C. Note the skin coloring.

    D. Complete an oral exam.

Correct Answer: B. Observe for abdominal pain, nausea or pallor, fatigue and tachycardia indicating symptoms of anemia.

Rationale: Because lead is absorbed into the blood, bones, teeth, and soft tissues and the half-life of lead is at least 1 month it can take more than a year to reduce blood lead levels.  Observe for nausea, vomiting, anorexia, constipation, abdominal pain and anemia along with muscle weakness and decreased physical growth.
