Good afternoon,
I have been asked to update out Employee Incident Report. This is our internal document that employee's fill out and return to HR. The nurse may add information, if involved. I am wondering if anyone can share a copy of their form. I have searched and haven't found anything here.
Also, does anyone use a "Witness report," for lack of a better term. Admin is thinking of creating an additional optional form that could be completed by any witness to the accident/incident. If so, would you be able to share this document with me?
