    The last question we worked on was fairly easy, this one you might find a little more difficult. Remember this is a national test, what your district does may not be what this question(or any other) is looking for. I believe 51% got this one correct.

    All students with disabilities are guaranteed certain rights under which federal law?

    A             The Individuals with Disabilities Education Act (IDEA/IDEIA)

    B             The Rehabilitation Act of 1973 (Section 504)

    C             The Americans with Disabilities Act (ADA)

    D            The Family Educational Rights and Privacy Act (FERPA)

    Correct answer = B The Rehabilitation Act of 1973 (Section 504)

    Rationale: All students with disabilities are guaranteed certain rights under Section 504 of the Rehabilitation Act of 1973. This law is a civil rights law that disallows disability-based discrimination in public schools. Students with disabilities affecting learning are guaranteed the right to a free appropriate public education in the least restrictive environment as well as related services under IDEA/IDEIA. The ADA guarantees the rights of all individuals with disabilities and particularly the right to physical access to public buildings. FERPA protects the privacy of educational records.

    Nursing documentation should be accurate, complete, timely, sequential, auditable, readable, retrievable and reflective of nursing practice. In order for an entry to be considered timely, the documentation should be done

    1. after the school day.
    2. contemporaneously.
    3. in the next 24 hours.
    4. by the end of the school day.

    B. At the time care is provided. This is one of the reasons for bedside charting. 

    A, C, D are incorrect. If charting is delayed, important details to document may be forgotten.

    The answer is B- contemporaneously.
    To be accurate we should be charting as the visit is occurring or shortly after.
    Waiting till the end of the day, after the school day, in the next 24 hours could cause you to forget specific facts that should be documented

    I’m going to mix things up a little and go with D- by the end of the school day.
    When you’re super busy you might start your note with some key thoughts but the final documentation might not actually be “completed” until the end of the school day.

    So, I guess many of you thought this question was easy. It shows that you were thinking about "best practice" and "standards".

     

    Kendra, I think you popped back into that place we called the "real world". Please, please, please.  Leave the "real world" at work! Do not bring it into the testing area with you!

     

    I was a school nurse for 34 years. I know about the real world. I am now going to show you what the real world "should" look like. A child comes in limping and says their foot hurts, you check them out and discover that they have some redness and swelling on the lateral side of their right foot, you document that, you call home and let their parent know their child's right foot hurts, it is red and a little swollen, you are applying ice, you think they can remain in school but if it continues they might want to contact their PCP. You document that. The child goes back to class and you document that and move on to the next child.

     

    I know our goal is to get kids in and out fast, but if you document at the time of the visit, not only is it the standard of care, but you are not staying over on unpaid time trying to remember if it was the left or right foot, hoping that you didn't miss anyone, and praying that you got everything right. Some nurses say they take notes. You could document in the time it takes to takes to take notes. You are not trying to remember which foot it was and you are moving all around saying 'well I was sitting here and he was sitting there so it must have been his right foot. (don't say you have never done that, I have).

     

    So all that was to say that the correct answer is B, and leave the real world out of the questions, lol.
