Equipping children for a lifetime of good health! What does that mean?

For children with chronic health conditions, it means helping them become the masters of their destiny – of learning to control their condition vs. the health problem controlling them! They already have the job of growing up and now have the dual task of learning to live with a chronic health condition.

The school nurse as care coordinator wears many hats. The first being instructor / guide / facilitator to help children understand their disease and medications or treatment along with teaching them critical self-care skills. But where do you find the time between your high-traffic health office and meetings for coordination of care and other-duties-as-assigned? Make time, take time or squeeze it in to have proactive, short sessions with the students early in the week and it may cut down on traffic later.

Other care coordination connections? With parents to ensure consistent care from home to school, with primary healthcare provider to ensure meds and treatments are correct, and with teachers and staff to make sure the child is safe at school. Have you ever drawn a school care coordination map? It's a good way to confirm all bases are covered and illustrates your many connections that benefit the child.
