School Nurse and School-Based Health Centers Collaboration to Address Social Determinants of Health

Looking forward to the in-person NASN Conference where we hope you'll join us for our session on "School Nurse and School-Based Health Centers Collaboration to Address Social Determinants of Health."  In our roles as nurse practitioners and pediatric nurses, we recognize that collaborative efforts among the school health team to address social determinants of health (SDoH) are required to best serve our students and communities. We will explore collaborative efforts between community-based school nurses and school-based health centers (SBHCs) to co-create solutions that address SDoH and social needs. The school nurse is a critical, irreplaceable member of the school health team. This role offers opportunities to influence change and work collaboratively with SBHCs in advancing the health and well-being of all youth. Co-located in schools, where youth spend the most time, interprofessional collaboration with SBHCs allows a partnered health care team to address SDoH, through access to medical services, immediate referral to SBHC services – and most importantly – care coordination led by the school nurse. Together, the school nurse and SBHC can advocate for additional resources to address SDoH-related factors associated with adverse health outcomes. The most efficient and sustainable school nurse and SBHCs partnerships collaborate in a school-wide approach with students, school staff and administration, parents, and community members, to work together to address complex SDoH and to benefit all students.
