    Hi All, here's a question off the exam guide. The answer is C but I've always been taught to round down not up for insulin, can anyone shed some light on this? Thanks!

    A nine-year-old student with Type I Diabetes presents to the school nurse to report that their blood sugar is high and they need to self-administer a correction dose of insulin. They have an individualized health care plan in place with parental signature, as well as signed written orders for short acting insulin daily at lunch and "as needed" for blood glucose readings >150 from their health care provider. Their meter reads 242 and the prescriber's order for determining the correction dose is:

    Blood Glucose - 150

    They have their insulin pen and asks the nurse to double check that they have dialed in the correct number of units. It is 10am and his scheduled lunchtime is at noon.

    The appropriate correction dose of insulin to administer would be:

    a. 2 units of Lantus insulin.

    b. 1.5 units of Lantus insulin.

    c. 2 units of Novalog insulin.

    d. 1.5 units of Novalog insulin.

    Thanks for sharing this question...the way the DMMP is written it states: 

    For injections: 

    • Half Unit (round down for less than 0.25 or less than 0.75 and round up for greater than 0.25 or greater than 0.75)
    • Whole Unit (round down for less than 0.5 and round up for greater than 0.5)

    Hey! 
    I found this reference for rounding doses. For 1/2 unit pens, which novolog should be, if the calculated dose has a 0.8 or 0.9 you round up to the next whole unit.

    Hello, 

    The correction is for 92 blood sugar divided by 50, which is 1.84 units. Since the eight is greater than five, you would round up; if it is less than five, you would round down. The correct dose would be two units.

    The other important piece to this question is knowing what the different types of insulin are and the appropriate times to use them. Be sure to study why correct answers are correct, but also why the wrong answers are wrong.

    Yes, you are correct.  There are many other factors to consider.
